Combining files from folder - "Combine" dialog box error

Suddenly, the dialog box for Combining files - where you select the table or tab you want - is broken, all the formatting, command buttons, etc. are gone and there is only an unformatted list of the table names shown.

Has anyone experienced this? I have rebooted my computer and dialog box is still the same. I can't imagine why it would be like this. I have not made any significant changes to my computer. I've not experienced this with any other dialog boxes.

I've never seen this before!

My two cents suggestions:

Does it happen ONLY in this query or does this window occur in other queries (in other workbooks)?

If it's only in this one workbook, something might be corrupt with the file. Try recreating the file to see if it helps.
If it is in ALL workbook queries, then I'd suggest you reinstall the add-in (or if you are in Excel 2016 then run the Office repair utility).
